چکیده

Osteochondral lesion of the talus (OLT) is a broad term used to describe an injury or abnormality of the talar articular cartilage and adjacent bone. A variety of terms have been used to refer to this clinical entity including osteochondritis dissecans, osteochondral fracture, and osteochondral defect. Whether OLT of the talus is a precursor to more generalized arthrosis of the ankle remain unclear, but the condition is often symptomatic enough to warrant treatment. Above one third of cases conservative treatment is not successful and a surgery is indicated. Several surgical options have been described including: debridement, isolated or combined with drilling, excision and curettage, abrasion arthroplasty, microfracture technique arthroscopically or not, internal fixation with screws, autologous chondrocyte implantation and implantation of osteochondral autograft using single or multiple cylinders of articular cartilage and subchondral bone. The optimal solution remains uncertain. Furthermore smaller lesions are symptomatic and untreated OCDs can progress, current treatment strategies have not solved the problem The goal of this study was to retrospectively evaluate the long-term results of 23 patients who underwent local osteochondral talar autograft for the treatment of OLT.

Osteochondral lesions of the talus: localization and morphologic data from 424 patients using a novel anatomical grid scheme.

BACKGROUND The primary aim of this study was to evaluate the true incidence of osteochondral lesions on the talar dome by location and by morphologic characteristics on MRI. Because no universally accepted localization system for talar dome osteochondral lesions currently exists, we established a novel, nine-zone anatomical grid system on the talar dome for an accurate depiction of lesion locat...

Osteochondral autograft transplantation with biplanar distal tibial osteotomy for patients with concomitant large osteochondral lesion of the talus and varus ankle malalignment

BACKGROUND Osteochondral lesions of the talus (OLTs) are amongst the most common foot and ankle disorders. Varus ankle malalignment causes stress concentration on medial side of the joint, resulting in OLTs and osteoarthritis. For large symptomatic OLTs (>10 mm), Osteochondral autograft transplantation is usually recommended. This article highlights biplanar distal tibial osteotomy as an approa...

Intermediate outcomes of fresh talar osteochondral allografts for treatment of large osteochondral lesions of the talus.

BACKGROUND Large osteochondral defects of the talus present a treatment challenge. Fresh osteochondral allograft transplantation can be used for large lesions without the donor-site morbidity associated with other procedures such as autologous chondrocyte implantation or osteochondral autograft transfer. The goal of this study was to prospectively evaluate the intermediate outcomes of fresh ost...

Arthroscopic management of talar dome lesions using a transmalleolar approach.

Surgical treatment of posteromedial talar dome lesions is frequently necessary for Berndt and Harty grade IV osteochondral defects and nondisplaced osteochondral fragments resistant to conservative modalities. When operative intervention is indicated, the approach and management can be complicated by the location and extent of the injury. The operative technique we advocate allows direct exposu...
